hi, mute issues

hi i got muted and gaged on my accounts
and as i wanted to find out why i found this (look at screenshot)
how could i get into a Name like that? i didnt Change it and its not even in my Name history?
and i was spamming a bit but why permanent?

can you look into this?

It isn't listing the correct date. The mute is very old.
