You must link your Steam account to the forums [LINK]
If you are on non-Steam, you will have to mention it on your post and follow the old ban appeal format:
Name: (The in-game name used during the time of ban)
SteamID: (The banned steam ID)
In addition to linking your Steam account, you may provide any information you know regarding the ban such as the events surrounding it.
Rules
Any concerns over admin conduct go beyond the scope of the ban appeal. They shouldn't be posted in the thread since they may violate rules #1 and #7. These sort of concerns should instead be forwarded to administration staff. All ban appeals will otherwise undergo periodic peer review by other staff members for verification of legitimacy and admin conduct. If administrative attention is immediately required, they may be contacted through PM or means such as Steam chat.
These are some valid examples which classify under rule #7:

Rules
- Do not be provocative, disrespectful, or abusive.
- Do not create more than one ban appeal for each ban.
-> If your ban appeal gets rejected, you may not create another ban appeal for that ban.
-> If you bought a new copy of CS:S and still cannot play, you may then appeal or contact me directly.
- Do not create a ban appeal on one's behalf. If they're unable to appeal due to difficulties with English then contact an admin directly.
- Do not double post. There's an edit button for a reason.
- Do do not bother staff members about your ban appeal. All ban appeals shall be dealt with in a timely fashion.
- Do not lie! This will greatly worsen your standing and may make you ineligible to ever play here again.
- Do not respond to other ban appeals unless you've been given prior permission or have something valuable to contribute.*

- If present during the infraction taking place. Offering additional information surrounding the ban.
- You may offer your opinion/take on any evidence provided. If the evidence is a demo, make sure to post any ticks which are relative to your opinion/claims. This is necessary so people know exactly what you're talking about.
